It's simple yet effective; works perfectly right after installation, but is very customizable as well. We love that we can add our own CSS to the form and email template. Reasonably priced, and the free version is a great place to start. I tried some other apps on Shopify and this was the only one that was straightforward. Kartik is also extremely responsive to any issues you may have!
